Lumara Home Care (Haven Care LLC d/b/a Lumara Home Care) is a licensed Health Care Service Firm serving Bergen County, New Jersey. We take the privacy of the people who trust us seriously, our clients, their families, and the caregivers who work with us. This policy explains what personal information we collect, how we use it, and how we protect it.
1. Who This Policy Covers
This policy applies to two groups:
- Clients and Families — individuals who contact us, complete an intake form, or receive home care services, as well as their family members and authorized representatives.
- Caregivers and Applicants — individuals who apply for employment or work for Lumara Home Care.
2. Information We Collect
From Clients and Families
We collect information you provide directly, including: name, address, phone number, email address, care needs and preferences, emergency contact information, and insurance or billing details where applicable. When you submit a contact or intake form on our website, we collect the information entered in that form.
From Caregivers and Applicants
We collect information provided through the employment application and onboarding process, including: name, address, phone number, email address, government-issued identification, employment eligibility documentation (Form I-9), Social Security number (for payroll and tax purposes), professional licenses and certifications, CPR certification, TB screening results, driving documents, and a passport-style employee photo.
Automatically Collected Information
When you visit our website, we may collect standard technical information such as your IP address, browser type, and pages visited, through cookies and analytics tools. See Section 9 for more detail.
3. How We Use Your Information
We use the information we collect to:
- Provide, coordinate, and communicate about home care services
- Process employment applications and manage caregiver onboarding
- Maintain personnel records as required by New Jersey law
- Send SMS text messages related to care coordination or caregiver onboarding (see Section 5)
- Respond to inquiries and requests
- Comply with applicable federal and state laws and regulations
- Improve our website and service quality
4. How We Share Your Information
We do not sell your personal information. We do not share or rent mobile phone numbers with third parties for marketing purposes.
We may share information only in the following limited circumstances:
- Service providers — vendors who support our operations (scheduling software, payroll processor, training platforms) under contractual obligations to protect your information
- Legal compliance — when required by law, court order, or regulatory authority
- Safety — when necessary to protect the health or safety of a client, caregiver, or the public
5. SMS Messaging Programs
Lumara Home Care operates two SMS text messaging programs.
Program 1: Caregiver Onboarding SMS
This program sends onboarding-related communications to caregivers, including links to the onboarding portal, document submission reminders, and application status updates. Consent to receive these messages is obtained through the signed employment application. Message frequency varies based on onboarding status, typically 2 to 5 messages per onboarding cycle. Standard message and data rates may apply. Reply STOP at any time to opt out.
Program 2: Client Care Coordination SMS
This program sends care-related communications to clients and their authorized family members, including caregiver arrival notifications, scheduling updates, and care coordination messages. Consent is obtained through the client intake form on our website. Message frequency varies based on your care schedule, typically 1 to 4 messages per week. Standard message and data rates may apply. Reply STOP at any time to opt out. Reply HELP for assistance.
Mobile phone numbers collected for SMS communications are never sold or shared with third parties for marketing purposes.
6. Data Retention
We retain caregiver personnel records for a minimum of seven years following separation from employment, as required by New Jersey Health Care Service Firm (HCSF) regulations. Client records are retained in accordance with applicable state and federal requirements. Other information is retained for as long as reasonably necessary to fulfill the purposes described in this policy.
7. Data Security
We take reasonable administrative, technical, and physical measures to protect your personal information from unauthorized access, use, or disclosure. No transmission over the internet or electronic storage is completely secure; we cannot guarantee absolute security, but we are committed to protecting your information.
8. Your Rights and Choices

9. Cookies and Analytics
